Multiple Teams · Important Information for Boys Basketball

This important information is for all high school athletes who intend to tryout for a boys basketball team.

Please read the following information:
All basketball athletes (grades 9-12) are required to be COMPLETELY CLEARED by the SMHS training staff before they can participate in any basketball related activity.  This includes students enrolled in either the 9th grade or JV/Var basketball athletic periods.

This means that a “NEW” physical examination must be performed dated after May 1, 2018.  All the physical paperwork along with all the SMCISD & UIL required paperwork must be signed and submitted by August 17.  (This date is for students interested in trying out for boys basketball.)

The SMHS training staff is now back on campus to receive any and all paperwork and clear athletes.  You may want to call them (512-749-1980) to see what times they are available to take paperwork.

If our training staff is not available when you try to submit your athletic paperwork, you may turn it in to Ms. Nan, our athletic secretary.

Please be advised that Ms. Nan is not responsible to check the physicals; therefore, if something is incomplete, the student or parent will have to return and take care of the issue.
The August 17 deadline has been established to prevent your son from missing any time when we start school.  As per SMCISD & UIL rules, any athlete MUST be COMPLETELY CLEARED in order to participate in a sport or an athletic period.
Locations that may be able to assist with the physical exams on a walk-in basis, if needed, are:
  • Live Oak Health has a walk-in clinic on Wonder World Drive
  • My Primary Care Clinic may also do walk-ins on Thorpe Lane

You can choose any clinic or doctor you would like to see your athlete; however, please make sure the physical is completed on the correct paperwork (see links below):

Very Important:

ALL PHYSICAL PAPERWORK MUST BE TURNED IN TO THE TRAINING STAFF DIRECTLY OR TO MS. NAN (athletic secretary in the SAC).  DO NOT TURN IN ANY PHYSICAL PAPERWORK TO A COACH, THE MAIN OFFICE, THE SCHOOL NURSE, or WITH ANY PAPERWORK RELATED TO SCHEDULING OR SCHOOL.  Every year, paperwork gets misplaced and students have to revisit doctor offices to get copies of physical examinations.

Our training staff can be contacted for all physical and mandatory paperwork questions at:  512-749-1980
